You have successfully completed the learning activity for Interprofessionaility: Team-based Care.

The University of Connecticut School of Medicine designates this Enduring Material for a maximum of 1.00 AMA PRA Category 1 Credit(s)™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

In order to obtain credits, you must take the post-test (10 questions), complete a series of questions that evaluate the CME activity and, finally, complete the registration form. This should take approximately 10 minutes.

The post-test requires a minimum score of 70. If you do not score a 70 or greater you have the option of repeating the test immediately or going back to the educational activity and then taking the post-test. There are no limits to how many attempts you have for scoring 70 or higher.
